We examined the association between BMI and waist-to-height ratio (WHtR) within and across populations of different world regions and quantified how well these two metrics discriminate between people with and without hypertension.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Methods<\/h3>\n\n\n\n<p>We used data from studies carried out from 1990 to 2023 on BMI, WHtR and hypertension in people aged 20\u201364 years in representative samples of the general population in eight world regions. We graphically compared the regional distributions of BMI and WHtR, and calculated Pearson&#8217;s correlation coefficients between BMI and WHtR within each region. We used mixed-effects linear regression to estimate the extent to which WHtR varies across regions at the same BMI. We graphically examined the prevalence of hypertension and the distribution of people who have hypertension both in relation to BMI and WHtR, and we assessed how closely BMI and WHtR discriminate between participants with and without hypertension using C-statistic and net reclassification improvement (NRI).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Findings<\/h3>\n\n\n\n<p>The correlation between BMI and WHtR ranged from 0\u00b776 to 0\u00b789 within different regions. After adjusting for age and BMI, mean WHtR was highest in south Asia for both sexes, followed by Latin America and the Caribbean and the region of central Asia, Middle East and north Africa. Mean WHtR was lowest in central and eastern Europe for both sexes, in the high-income western region for women, and in Oceania for men. Conversely, to achieve an equivalent WHtR, the BMI of the population of south Asia would need to be, on average, 2\u00b779 kg\/m<sup>2<\/sup> (95% CI 2\u00b731\u20133\u00b728) lower for women and 1\u00b728 kg\/m<sup>2<\/sup> (1\u00b702\u20131\u00b754) lower for men than in the high-income western region. In every region, hypertension prevalence increased with both BMI and WHtR. Models with either of these two adiposity metrics had virtually identical C-statistics and NRIs for every region and sex, with C-statistics ranging from 0\u00b772 to 0\u00b781 and NRIs ranging from 0\u00b734 to 0\u00b757 in different region and sex combinations. When both BMI and WHtR were used, performance improved only slightly compared with using either adiposity measure alone.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Interpretation<\/h3>\n\n\n\n<p>BMI can distinguish young and middle-aged adults with higher versus lower amounts of abdominal adiposity with moderate-to-high accuracy, and both BMI and WHtR distinguish people with or without hypertension. However, at the same BMI level, people in south Asia, Latin America and the Caribbean, and the region of central Asia, Middle East and north Africa, have higher WHtR than in the other regions.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Funding<\/h3>\n\n\n\n<p>UK Medical Research Council and UK Research and Innovation (Innovate UK).<\/p>\n\n\n\n<p><strong>Zusammenfassung<\/strong><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Hintergrund<\/h3>\n\n\n\n<p>Adipositas kann anhand des BMI (der auf Gewicht und Gr\u00f6\u00dfe basiert) sowie anhand von Indizes f\u00fcr abdominale Adipositas gemessen werden. Wir untersuchten den Zusammenhang zwischen BMI und dem Verh\u00e4ltnis von Taille zu K\u00f6rpergr\u00f6\u00dfe (WHtR) in und zwischen Populationen verschiedener Weltregionen und bewerteten, wie gut diese beiden Messgr\u00f6\u00dfen zwischen Menschen mit und ohne Bluthochdruck unterscheiden.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Methoden<\/h3>\n\n\n\n<p>Wir verwendeten Daten aus Studien, die zwischen 1990 und 2023 zu BMI, WHtR und Bluthochdruck bei Personen im Alter von 20 bis 64 Jahren in repr\u00e4sentativen Stichproben der Allgemeinbev\u00f6lkerung in acht Weltregionen durchgef\u00fchrt wurden. Wir verglichen die regionalen Verteilungen von BMI und WHtR grafisch und berechneten die Pearson-Korrelationskoeffizienten zwischen BMI und WHtR innerhalb jeder Region. Mithilfe einer linearen Regression mit gemischten Effekten sch\u00e4tzten wir, inwieweit die WHtR bei gleichem BMI zwischen den Regionen variiert. Wir untersuchten grafisch die Pr\u00e4valenz von Bluthochdruck und die Verteilung von Personen mit Bluthochdruck in Bezug auf BMI und WHtR, und wir bewerteten, wie gut BMI und WHtR zwischen Teilnehmern mit und ohne Bluthochdruck diskriminieren, indem wir die C-Statistik und die Netto-Reklassifikationsverbesserung (NRI) verwendeten.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Ergebnisse<\/h3>\n\n\n\n<p>Die Korrelation zwischen BMI und WHtR reichte von 0-76 bis 0-89 in verschiedenen Regionen. Nach Anpassung f\u00fcr Alter und BMI war die mittlere WHtR f\u00fcr beide Geschlechter in S\u00fcdasien am h\u00f6chsten, gefolgt von Lateinamerika und der Karibik sowie der Region Zentralasien, Naher Osten und Nordafrika. Am niedrigsten war die mittlere WHtR f\u00fcr beide Geschlechter in Mittel- und Osteuropa, f\u00fcr Frauen in der westlichen Region mit hohem Einkommen und f\u00fcr M\u00e4nner in Ozeanien. Umgekehrt m\u00fcsste der BMI der Bev\u00f6lkerung S\u00fcdasiens im Durchschnitt 2-79 <sup>kg\/m2<\/sup> (95% CI 2-31-3-28) bei Frauen und 1-28 <sup>kg\/m2<\/sup> (1-02-1-54) bei M\u00e4nnern niedriger sein als in der westlichen Region mit hohem Einkommen, um eine entsprechende WHtR zu erreichen. In jeder Region stieg die Pr\u00e4valenz von Bluthochdruck sowohl mit dem BMI als auch mit dem WHtR. Modelle mit einer dieser beiden Adipositasmetriken wiesen f\u00fcr jede Region und jedes Geschlecht praktisch identische C-Statistiken und NRIs auf, wobei die C-Statistiken zwischen 0-72 und 0-81 und die NRIs zwischen 0-34 und 0-57 in verschiedenen Kombinationen von Region und Geschlecht lagen. Wenn sowohl der BMI als auch der WHtR verwendet wurden, verbesserte sich die Leistung nur geringf\u00fcgig im Vergleich zur Verwendung eines der beiden Adipositasma\u00dfe allein.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Auswertung<\/h3>\n\n\n\n<p>Der BMI kann mit m\u00e4\u00dfiger bis hoher Genauigkeit zwischen jungen Erwachsenen und Erwachsenen mittleren Alters mit h\u00f6herer und niedrigerer abdominaler Adipositas unterscheiden, und sowohl BMI als auch WHtR unterscheiden zwischen Menschen mit und ohne Bluthochdruck.
